Summer Health Advocate Graduation
42 new Health Cabinet members completed their certification at our summer cohort graduation ceremony. Graduates represented 14 Chicago neighborhoods.
Back-to-School Health Fair
Over 300 families received free health screenings, school physicals, and resource referrals at our annual back-to-school fair in partnership with Sinai Health System.
Navigating Chronic Disease: A Patient Forum
Community members living with diabetes, hypertension, and heart disease shared their experiences and connected with local care navigators and peer support groups.
